While our doc was doing a laparoscopic prostate removal, he enlarged the umbilical port and wants to charge for an umbilical hernia repair. What is everyone else's thought on this ? I feel like if we are creating the defect, we shouldn't charge this. Coding as 55866,38571,49592.

The same physician cannot bill for repairing something they created. I would not bill the 49592 if the surgeon created the hernia.
If an outside physician (not part of your group) repaired the injury, that would be billable.
