Is 67028-51-RT unbundled from 67210-RT?

No, just that the vitreous was removed and replaced with a Balanced Salt Solution plus a pharmacologic agent. The vitreous is what keeps the eyeball round but some of it must be removed in order to make room for medication.

CPT code 67028 is a (separate procedure) meaning if it is performed with other procedures it needs a 59 or "X" modifier.
It is often a component of a more complex procedure...but if it's performed with an unrelated service it may be reported using a modifier.
