We have a general surgeon that does quite a lot of Nissen procedures and also does quite a bit of unlisted laparoscopic procedures. We are having trouble setting our RVU's and pricing for these unlisted procedures.

Can anyone give me hints on how they handle this situation?

Look at the open procedure

We usually look at the equivalent open procedure, and base our fee for the unlisted lap procedure accordingly.
