Unregistered "Patient" for Pre-Natal Tour

Our Neonatologists do not have an office. They are hospital based only. They have expectant mothers referred to them to talk about possible scenarios and tour the NICU. The mother is not a registered patient at the hospital at the time, and are not registered as a patient of my Neonatologists. How can we charge for this service? I cannot find any similar situation, and the medicare guidelines say the patient must be registered at the facility. They are not really a patient, and are not registered. Help!
