Will the word "unremarkable" count towards an area for the exam?

Example-

Head- unremarkable
Eyes- unremarkable
Ears- small lesion on outer area
 
1995 vs 1997 guidelines

Depends on whether you are using 1995 or 1997 guidelines.

For 1997 guidelines you have to specify the bullets, so this kind of general statement will not count.

For 1995 guidelines, you can document "normal" for a given system or body area if there are no specific findings.
