What verbiage should be used when a physician ,in the same practice, has to review and sign charts for an physician who is severely injured and unable to sign.

I was not present at the time the services took place but based on the medical documentation by Dr. Jones, I agree with the assessment and medical decision making noted. Dr. Jones is unable to sign this chart timely due to an extended leave of absence.
