Indications

PAD (peripheral artery disease) (CMS-HCC) [I73.9 (ICD-10-CM)]
Conclusion

Patient with advanced PAD and left foot rest pain and redness. Started on IV heparin with nitroglycerin applied to left foot. CTA aorta with runoff revealed severe ostial disease in left common iliac, external iliac, and common femoral artery. Attempted getting access into left common femoral artery. I was able to get access but was not able to thread the wire due severe calcification and possible occlusion proximal to the access needle. Attempted crossing from the right radial artery access to perform aortogram which was unsuccessful due to severe tortuosity. Ascending aortography was performed to delineate origin of descending aorta. Procedure was aborted. TR band was placed on right radial artery access site.

Aortogram revealed patent ascending aorta without significant dilatation or aneurysm formation.

thanks in advance
