In need of some help , would anyone know how to code the following?

The patient was placed in reverse-Trendelenburg position and local anesthesia was instilled in the skin overlying the access site.
• The left GSV was attempted to be was punctured using ultrasound guidance but guide wire was unable to be introduced through the needle. Multiple attempts were made but vein became significant spastic and unable to be accessed despite several attempts at different levels. Topical nitroglycerine was applied to distal GSV but still unable to dilate vein for access. In view of this procedure was aborted.
