this is short of it from his dictation. He has all his dictation correct

My doctor upgraded to BIV pm with new RV lead implant, temporary pm wire placement and removal.

patient previous had single chamber implanted with an epicardial lead . The patient presents for a pm generator change out and addition of RV pacing lead.

explanted single device
implanted pm generator

lead information:
RA lead port is plugged
New RV lead
New LV lead

temp epicardial wire removed

Would the codes be 33233, 33235, 33225 and 33207 or 33208

Do we follow the leads inserted when coding or do we follow what system patient leaves with?

Any guidance and reference is appreciated. Thanks
 
yep, the 33214 is designed for this situation, includes removing old generator, inserting new lead, and inserting new generator. then the 33225 for the LV lead. unbundling it would be over coding
