Can a provider diagnosis a patient with an Upper Respiratory Infection if the physical examinations shows everything as normal but the patient claims to be having some of the symptoms such as runny nose or cough?

The provider can diagnose a patient with any dx at anytime. It is not up to us to question how or why a diagnosis has been determined, as long as it is documented then we can code it. However it must be documented not just a code number.
