Do you charge a facility fee and physician fee when charging for I & D , lacerations, etc. along with the EM code for facility and physician. Do I enter 4 charges or just 3 charges? Thank you

If you are coding both pro and fac for an ER, you code the pro E&M and I&D. You do the same for the facility.
