We are an independent urgent care facility. I was told to use 59 modifier on all our procedures and labs with a 25 on the office visit. We are level coding because we are independet site.
 
you use the 59 and the 25 when it is appropriate to do so based on the documentation. You cannot make a blanket rule like this because it may not be the correct action on any one individual claim.
