I need help coding this.

A patient is admitted after being found unresponsive at home. The physical examination reveals weakness and aphasia that does not resolve by discharge. The physician documents CVA as the final diagnosis and the patient is transferred for rehabilitation.

I'm coding for a inpatient facility

If this is the facility the patient is being discharged from then code the CVA. If this is the rehab facility then code the rehab V code first listed followed by the stroke residual.
