when billing a 80305 urine drug screen at the time of ob visit what is the dx you typically use? If a patient who had a positive drug screen at initial visit but all the rest have been negative what dx should be assigned, the office is wanting to assign the r82.5.
 
If the provider documented abnormal results the I agree with O99.89 and R82.5 if the provider does not document that the results were abnormal then there is nothing to code. A coder cannot code based on lab results, the provider must provide an interpretation.
