Wiki Urine total protein/creatinine ratio

We are performing an 84156 total protein urine and 82570 total urine creatinine that together as a UTPCR or urine total/protein creatinine ratio. The 82570 is done to assure we are are testing on a 24 hour collection. I can find info on the 82570 but I can not locate a policy for the 84156. We often do this to evaluate patients with renal disease and or diabetes. I can not find any information regarding covered dx, frequency, etc for the 84156 for Medicare part B. Can any one point me in the right direction?
