How would I code this? I have dx of listed on the consulation:
N20.0 for Calculus of kidney- which is why the patient came to the clinic for a followup
N39.46-Mixed incontinence
R35.0-Frequency of micturition
R31.9 Hematuria
R35.1 Nocturia

On the physicians order he has
OAB- N32.81

My question on this is that when I code the mixed incontinence N39.46 the OAB N32.81 is a Code also with it. The other codes N20.0 for Calculus of Kidney, R35.0 for Frequency of micturition, R31.9 Hematuria, and R35.1 Nocturia are all symptoms of the OAB N32.81 and actually has an Excludes 1. Do I just code the Mixed incontinence and the OAB if the patient came in for a follow-up for Calculus of the kidney. Maybe I am just confusing myself but not sure what to do on this?

Your coding instinct seems correct and follow those Excludes 1 rules because the insurance payers sure do. Did you add dx Z09 for follow up as last dx code on claim? I hope the med record says follow up too
