Can anyone tell me the documentation requirements when billing 76942

Because this is for guidance, The documentation would haveto say something along the lines.... Ultrasound was used, xxx was identified by ultrasound guidance. It would'nt have to say much just that it was used. Be sure to check the codes you are using it with. Over the past few years more and more codes include the guidance.
