I work for the VA and was dinged on the National Quarterly audit for not using 25 modifier on a preventive medicine code.
Scenario: Patient was seen for preventive med but also had a separately identifiable E/M and vaccines. I entered modifier 25 on the E/M as described in coding guidelines, but again was dinged for not also adding to the preventive med code.
