Use of PA's as scribes for physician

Does anyone have any guidlelines on using your PA as a scribe for inpatient consultations?? Doing a split/shared visit seems to be too much duplication as we only have one cardiothoracic surgeon right now. However, if I bill only using the PA's NPI number then I take a hit on the reimbursement. Does anyone have any suggestions???? ANY HELP/GUIDANCE WILL BE GREATLY APPRECIATED!
