use of VS in diagnosis

When the doctor gives the diagnosis as: "BPH vs PRostate Cancer" or "TIA vs CVA," what code should be used?
please help
 
Technically, if the doctor uses "vs" in the diagnosis, neither can be coded. It essentially makes them "rule out" diagnoses. As for the TIA vs CVA though, these are such acute events that unless the patient is in a hospital setting, there is not going to be a time where you are going to code these as anything but a history of UNLESS there are residual late effects. Hope this helps. Anyone disagree?
