If an Oncologist has an E/M visit with a patient on Day 1, and they determine that the patient is a candidate for chemotherapy that will present to the chemo infusion clinic on Day 14, but on Day 11 they write the specific treatment plan with dosages for approval by the pharmacy can they report that chemo plan creation on Day 11 with codes 99358 and 99359?

Not with Medicare. Those codes are non-payable by Medicare effective 2023.
