Hi All,

Is anyone out there in a private office using compounded medications versus the commercial? Even though the formulary is exactly the same, do we need to bill with the unclassified drug code of J3490?

I am confused on this as I look in the HCPCS .... Hyalgan for instance....we are using a Hyaluronic Acid Sod compounded by a Compounding Service for our office. They are telling me it is the exact same formulary as Hyalgan; however, it is not the actual brand "Hyalgan" with corresponding NDC code. Can we use J7321 as the HCPCS says: 'Hyaluronan or derivative, Hyalgan or Supartz...'?

I am reading too much into this...and should we be billing out the J3490 versus the J7321?

Any and all comments are much appreciated.
