I just wanted to ask in what cases other coders are using cyanosis clubbing and edema as elements of ROS or PE. And what is the rationale behind it. Is it because cyanosis is looking at the coloring of the skin or that clubbing has something to do with how the bones have formed, I would like to understand it fully if possible. Thanks.
 
Who is reporting?

Whether you use any verbiage as ROS vs PE is dependent upon who is reporting it.

Was this a response BY THE PATIENT to a query about symptoms - YES? - then it's ROS

Was this a direct observation BY THE PHYSICIAN - YES? - then it's part of the exam.
