This is an inpatient chart. The provider only documented "UTI with SIRS" under Assesment. Under Plan they documented IV fluid hydration, ABX tx, and F/U on blood and urine cultures. Theres is no mention of bacteria or what is causing UTI. Do I code this as R64.10. Is the this statement of UTI with SIRS enough to code the SIRS as due to UTI?
