Wiki V Codes for Follow Up Exams

I'm struggling with choosing the right code for follow up office visits. If a patient comes in for 6 month follow ups for chronic condition(s), such as hypertension, which V code should I use?

What's throwing me off is the heading for V67 codes: "surveillance only following completed treatment"...which indicates the condition has been resolved (healed). I thought it would be V67.9, but I'm second guessing myself because that's not really a good fit. So then I thought maybe V72.85? Just not sure...
 
You do not always have to use V codes for follow-up visits. If a patient comes in for a chronic condition that is currently being treated (i.e. hypertension, diabetes) you would code for the condition.

Read the Introduction at the beginning of the V codes in your ICD book to help you understand when to use these codes. If you don't have an ICD book (I know many companies are no longer buying actual books) you can find the ICD coding guidelines at the CDC or WHO websites.
