If a patient is seen for a well visit with immunizations. The parent signs a waiver saying they want the immunizations then later after the vaccine is drawn up, for whatever reason, chooses not to receive the vaccine can we bill for the wasted vaccine?

I think you'd be able to code it as if it was given but then add a mod 52 which states that the service was reduced or eliminated (not carried out) and then add the dx V64.05 which states that vaccine was not carried out because of parent refusal.
