I am new to the world of CPT coding. I have been a nurse for many years and understand what a nurse needs to document for vaccines/immunizations and IM injections, but when reviewing records for my insurance company to determine if they (provider's ofc) have properly documented the shot so that they can get reimbursed - I'm unclear about that.

Perhaps it differs from insurance company to insurance company, looking for general guidelines. References would really be appreciated -I'm not even sure where to start looking, as the CPT book doesn't really say.
