Patient starts on 1,000mg of Vancomycin HCL (J3370 x 2) and is stepped down to 750mg
(J3370 x ?)

reconstituted "shelf life" is 48 hours and Vanco is purchased in either 1,000mg or 500mg vials -

for the above referrenced patient, Vanco is purchased in 500mg vials and only 250mg of the second vial is used (1 1/2 vials used per day) and the remainder of the vial (250mg) is used for the next days infusion for the same patient.

How would you bill for 1/2 a 500mg vial, especially when your billing code is indicated in 500mg (J3370)?

I can't bill for 2 units on the first day and then 1 unit the following day, because the patient didn't receive 2 full units the first day and received more than 1 unit the second day, and you can't bill for fractions of a unit. :confused:
HELP PLEASE!
