Vancomycin and Ceftazidime

My provider has started using a couple of new drugs for our office and I am having trouble with Medicare. Does anyone bill out for Vancomycin 1mg/0.1ml and Ceftazidime 2.25mg/0.1ml for the DX of acute endophthalmitis? Has anyone been able to find an LCD or NCD for these drugs? I appreciate any help you can give me.
