Wiki Vascular Bone Graft for Kienbock's disease

One of our surgeons did a vascular bone graft with a flap from the distal radius on a patient with Kienbock's disease. I am not sure how to code this, whether I should code the bone graft with a 22 modifer along with a separate flap code, or does anyone have any other ideas?
