Surgeon's description of operation:
Revision of right lower extremity arterial bypass graft with 2 separate cadaveric vein interposition bypasses, first one extending from the common femoral artery to the proximal existing vein bypass and the second bypass from the distal graft to the graft just above the distal anastomosis which is below the knee.
He coded 35881-RT with 35681. Medicare denied 35681 because it is an add on code for a procedure that we did not bill. Is there another code that will work for the bypass graft? I am not a coder and I am trying to learn but this is way way over my head.
