Wiki Venogram Denial not documented

Bilateral Venogram is being denied as not documented by Optum, anyone has the same issue?

I have run into this issue with Optum as well, both with venogram denials as not documented and a sudden increase in denials of diagnostic cerebral angiograms (there is overlap in the two at times for the providers I work for). I have recently been submitting appeals back to Optum (typically found on the results letter they send back) with the op note (like you attached to you post) marked up with where the codes are documented. I also include with that a brief letter explaining that the denial is incorrect, and that I have outlined where the Venogram is clearly dictated. I can message you privately a redacted version of what I've found some recent success with if you'd like...

Clearly it's right there in the documentation! Only Optum seems to miss it...
