If a pre-existing left brachial venous access is used and a venogram (75820-59) was done showing chronic occlusion in the left brachiocephalic vein and a venoplasty (37248) is done in the left brachiocephalic. Is 36005 appropriate to use for the injection or should a 52 modifier be added to indicate that only part of 36005 was done?
