Can anyone tell me: does 454.0 (venous stasis with ulcer) requier a second code for the ulcer (707.1X)? Or is it OK used alone?

It does not instruct to use a second code and the code specifies to the lower extremity any part so I would say no.

I would only use the venous stasis ulcer code(s) (454._).
