Vermont Medicare's MAC is NHIC. I cannot find anything on that website that indicates they have been denying G0431 with QW modifier incorrectly, prior to April 1. After April 1 QW modifier is no longer needed. However, the only documentation I can find that these claims are being processed incorrectly is through Highmark, which services Wisconsin. I need specific documentation that states they are processing issues with this code.

Can anyone point me in the right direction? And not just the CMS.gov website as I have had no luck there. I apparently have not figured out the magical words to search. I appreciate any and all help.
