Viral lower respiratory tract infection- what is the ICD code for this? Is it J22?
Do I add cough or is that included as a symptom? I have been deleting that.
Do I add B97.89 for virus? Even when virus is not known?

J22 is an unspecified acute lower respiratory infection. You wouldn't use B97.89 - it wouldn't add any descriptive clarity for the patient's condition.

Cough would be a symptom of the respiratory infection so that I wouldn't add a separate code for that.

Thank you that's exactly the answer I wanted to hear!
