I'm new to opthalmology and have a question regarding medical necessity of a visual fields test. My doc is using pre-glaucoma as a diagnosis (H40.003 to be exact) and I'm receiving denials from Medicare for medical necessity. This ICD-10 IS listed on the LCD as a covered diagnosis. Any thoughts? What would bring the provider to a "pre-glaucoma" diagnosis?

Possible alternative

You may want to try H40.023, open angle with borderline findings, high risk

What the doctor may see that points to risk of glaucoma is slightly elevated intraocular pressures and or changes in the optic nerve head appearance.
