I am needing help with a diagnosis. A patient came in for her yearly pap and the nurse took her vitals. The nurse practitioner came in to do her pap and they realized it was too soon, it hadn't been a year. One, can I charge for a nurses visit, 99211 for only doing vitals? Two, what diagnosis would I use? The patient was perfectly healthy. Thanks in advance for any input!
