doc orders a basic metabolic panel plus gave vitamin D shot.

problem - cant see why vitamin D was given is this a normal practice when pt change HTN meds..

I believe vitamin D deficiency plays a casual role in hypertension. Personally, I would have the doctor enter a dated and signed addendum to indicate why the vitamin D shot was given. Perhaps there was a recent lab result with a low vitamin D level, but without being directed to that information no one reading the chart will know.
