Wiki waiver fee schedule

Sounds like it is what we call a Financial Hardship Waiver, or sliding fee schedule. The patient's bill is reduced according to the income statement that is provided on the waiver. Then if they qualify we can reduce their bill according to family size and income based on the data that can be found on the HHS Poverty Guidelines website. http://aspe.hhs.gov/poverty/index.cfm
It is illegal to reduce patient balances without documentation justifying this action. We ask them to fill out the form and provide a copy of the last 4 paycheck stubs or last year's tax return. The form asks how many members in the family and what their expenses are. We also request they submit the form and information within a 30 day time limit. Hope this helps.
