Does anyone have or know where I could find a copy of a waiver form for commercial pay patients to sign for non covered services?

We took the set up of the Medicare ABN and customized it for the office so we could write in the name of the insurance company and other pertinent info. Use the "official ABN" for Medicare patients and your office one for others.
