One of our providers wishes to treat common warts with Gardasil injections.
Gardasil does not currently have a J code.
Only 90649/90471 related to vaccination series for HPV.
Would 17110 be appropriate for benign lesion destruction in this instance?

Perhaps you are misunderstanding what your provider wants to do. There are no clinical studies, but there is anecdotal evidence that getting the gardasil vaccine improves hyperkaratotic warty lesions/condyloma. The gardasil is NOT injected into the warts, but given as a regular vaccine, x3.

My derm wanted to do the same thing. There is a JAMA article about treating resistant warts with HPV. Regardless, there is not a HCPCS code specific for the medication; only the medicine codes for the vaccine. The vaccine would not be appropriate. If he is only injecting, I would use 11900/11901 + J3490. The J3490 would probably not get reimbursed. If he is also freezing or using a laser I would only bill 17110 and not bill 11900/11901/J3490
