1. Washout / debridement of left interstim pocket

2. Closure of Left and right interstim incisions


Procedure

The patient was taken to the operating room and placed prone on the operating table. Pre-operative antibiotics were administered. Bilateral lower extremity SCDs were placed. After induction of anesthesia the patient was maintained in prone position.


The left incision ws debrided sharply with a 15 blade. It was then irrigated copiously with bacitracin solution. Figure of 8 sutures were placed in deep dermal tissue w/ 3-0 polysorb. The skin was then closed with 2-0 nylon in interrupted horizontal mattress. The edges of the partially separated but not opened Right wound were also buttressed with 2-0 nylon horizontal mattress.



A revision code? I'm lost on this one! Help please :)

13160 - Secondary closure of surgical wound or dehiscence, extensive or complicated
Lay Description
The physician secondarily repairs a surgical skin closure after an infectious breakdown of the healing skin. After resolution of the infection, the wound is now ready for closure. The physician uses a scalpel to excise granulation and scar tissue. Skin margins are trimmed to bleeding edges. The wound is sutured in several layers.

12020 - Treatment of superficial wound dehiscence; simple closure
Lay Description
There has been a breakdown of the healing skin either before or after suture removal. The skin margins have opened. The physician cleanses the wound with irrigation and antimicrobial solutions. The skin margins may be trimmed to initiate bleeding surfaces. Report 12020 if the wound is sutured in a single layer.

12021 is a third choice, but include wound packing which you did not mention.
