Wiki Weird Vasectomy Question

I have a strange vasectomy conundrum that I need some coding guidance on, please.

Patient presented to the office for a bilateral vasectomy. We completed the right side, but the left side was not able to be safely isolated from the thick spermatic cord and the left vasectomy had to be completed in the OR.

It should be noted here that these took place on different dates of service.

I am thinking we should only code for the OR encounter and not each one separately. Modifiers? Thoughts?

bill the first one with the -RT modifier with the date and POS for the office.
When you go in to do the left side, bill -LT (different date and POS). If you're within the global days, use the -58 for a staged, related procedure and plan to send notes.
