Hello, for the elements of the IPPE, such as the review of the benficiary's functional ability and level of safety, end of life planning & assessing daily living; if a HRA (helth risk assessment) form is given to the patient to be filled out and the physician reviews it is it enoigh to say in dictation "review & noted HRA, document scanned"? Does that justify all requirements if there was to be an audt?
