Wiki Well care VS Sick E/M = can be charged on same day?

Please help....
The provider wants to do two things during a patient's visit:
1) Well care check-up wherein the provider can administer the immunizations, flu vaccines, etc.
2) at the same time, pt was diagnosed with asthma-related problems, therefore, was treated with asthma related treatment.

Can a provider do this and charge 2 separate E/M (ex: well care 99381 DX: v20.2 plus the immunization/admin of the drugs and 99213 DX 493.90) on SAME day? if so, is there a modifier to be used on the E/M? which E/M?

well

Yes, and mod -25 goes on the OV. You cannot use to justify level of OV w anything which was examined during the physical. It must be a separate exam whose documentation will stand on its own.

That's the part some people forget. Your level 3 may end up as a 2.
