Would you change to abnormal if the patient presents with a known problem and not the physician finding at the exam.
 
I may be the only one, but I'm lost. What are you using PE to stand for? That could mean a number of different things. Physical exam? Pulmonary exam? Podiatry exam? Many of my providers use that abbreviation to mean Pulmonary Embolism, so you can understand my confusion.

A little context helps when asking these sorts of questions. It's hard to know where to begin when the question is this unclear.
