Hello,

My co-biller/coder and I are having a difficult time getting information across to our providers regarding wellness/preventive visits as well as miscellaneous physical exams.

For DOT Physicals, sports physicals & miscellaneous physicals (i.e. adoption physicals), is "scanned forms" suffecient documentation for the physical to be billed as wellness to insurance? The forms do not have our clinic name. Is it common for providers not to document these types of physicals at all?
