If the doctor sees a patient thinking the visit was for a wellness and the patient does not have Wellness benefits available, is it ok for the physician to bill a regular office visit? And if so, can you bill a 99213 with Z00.00 if there is no other diagnosis?
 
If the intent for the visit was for preventive care, you cannot bill an office visit (i.e. sick visit) without a chief complaint. Billing an E&M visit with Z00.00 will likely result in a denial. Make sure you schedule, room and inquire as to the chief complaint before the provider sees the patient and launches the correct documentation template in your EHR.
