I printed a response from Dr. Pechacek MD where he answered this question for the shoulder. The procedure is performed in an attempt to save the bone from AVN if possible. Dr. Pechacek suggested the unlisted code.

I have not seen this procedure around the knee, but I have never seen a code for it either. So I think unlisted is the way to go.
